A range of personal risk assessments. Personal assessments are not scored or colour coded but they are usually subject to review and the default is 30 days and usually co-ordinated with care plan review but sometimes more or less often.

General Health and Well-being

When assessing the general health of a resident it is about identifying what health related conditions that they been diagnosed with and what associated risks there are to them in relation to the illness.


General Risk Assessment

General risk assessment, to be used on an ad-hoc basis for non-standard risks. Template can be used multiple times for different risks


Living with Dementia

Measurement of a resident’s cognitive function; key to detecting dementia, delirium and early onset of Alzheimer’s. The template includes cross-references to safe environment, choice and control, reducing harm/violence/aggression, refusal of prescribed medication, well-being, independence, sexual relationship development and adherence with policies and procedures.


Medication Administration

A control document for medication for the resident.


Nutrition

A control document overseeing ongoing assessments of a resident's nutrition and hydration needs. Nuagecare, by default, uses the Malnutrition Universal Screening Tool (MUST) each time a resident’s weight is recorded.


Personal Bed Rails Assessment

Mandatory in the case of a red colour coded risk assessment. A full analysis of why bed rails are in use for an individual resident and a series of checks and balances to ensure they are being used correctly.


Personal Care

A highly individualised document outlining general personal care needs and cross-references to more detailed documents for mobility, oral health, general health and elimination.


Personal Dehydration Assessment

Mandatory in the case of a red colour coded risk assessment. A series of checks and balances to ensure a resident vulnerable to dehydration is well monitored and supported.


Personal Emergency Evacuation Plan Assessment

Mandatory in the case of a red colour coded risk assessment. An individual personal emergency evacuation plan tailored to the needs of a resident.


Personal Falls Assessment

Mandatory in the case of a red colour coded risk assessment. A series of checks and balances to ensure minimisation of the risk of falling for an individual resident.


Personal Malnutrition Assessment

Mandatory in the case of a red colour coded risk assessment. A series of checks and balances to ensure a resident vulnerable to malnutrition is well monitored and supported.


Personal Moving and Handling Assessment

Mandatory in the case of a red colour coded risk assessment. An individualised series of guidelines for moving and handling a resident.


Personal Oral Hygiene Assessment

Mandatory in the case of a red colour coded risk assessment. Guidelines, checks and balances for oral hygiene for a resident.


Resident Hoisting Record

An oversight document to give carers a quick reference point for usage of a hoist for a resident.


Safe Environment

An oversight document ensuring everything is being done to ensure the care home is a safe environment for a resident.


Self-Medication Assessment and Permissions

Optional, implemented in the case of a resident opting for self-medication. This document is normally completed and then printed, signed, scanned and uploaded to the document management system. It is completed by a senior staff member who is competent to administer medications.


Self-Medication Review

Optional, implemented in the case of a resident opting for self-medication. Usually carried out weekly, return from hospital or change in clinical condition.
